Special Consulting Forms:

The Request for Special Consultant Pay form is submitted directly to HR Programs.  Please do not send to Budget or Payroll

Address Change Forms:

Please complete BOTH forms above (On the Employee Action Request form, complete only parts C,F & I) and submit to HR Programs

Withholding Change Form:

Please complete Sections C,E & I and submit to HR Programs

Direct Deposit Form:

Please complete and return to HR Programs.  We are currently located in Solano Hall, Room 1123. 

Please keep in mind:

  1. You should check with your financial institution to be sure that the numbers at the bottom of your check are what they use for EFT.  NOTE: A voided check is not needed for processing.
  2. You need at least 40 leave credits in order to enroll in Direct Deposit.  (*Faculty being the exception to this rule)
  3. Student Assistants are not elligible (due to #2 listed above)
  4. There are some pay periods in which your check will not go into your account by the first day of the month, because the pay-period ends on the 1st or after; Or - due to the Winter Holiday, when the direct deposit doesn't go in to the accounts until the 4th.
